  • Your name
  • Your AAMC ID
  • Your return e-mail address
  • The name of your designated dean's office

Responses to requests received without this information may be delayed.

Be very specific when describing your problem. Please include:

  • Exact wording of error messages (cut and paste if possible)
  • Information about the type of computer being used (manufacturer, processor, amount of RAM, available hard disk space)
  • Operating system (e.g., Windows 2000, Windows XP)
  • Type and version of your browser
